· Candidates should have completed their 12TH class form PCM or PCB with 45% as the main subjects from well recognized Board of India.

Domicile:
· Candidates who have passed the qualifying exam from UP or candidates whose parents are UP domiciled are eligible to apply.
· Candidates whose parents have domicile of UP state, but he/she has completed his/her studies from outside of UP state are also eligible to apply.
· If candidate parents are neither UP domiciled nor candidate has passed the qualifying exam from UP. Then these candidates are also eligible to apply for 15% seats.
No prescribed age limit for the courses covered under UPSEE.

· Double click on the link, a registration form will open.
· Read the instruction those are given in the portal.
· You need to fill your general details like name, father name. You need to validate your Aadhar Card with the application form.
· After registration, fill the form carefully.
· Now upload your photograph and signature in the prescribed manner.
· Make payment of form fee using net banking or card payment.
· Save the receipt for the future usage.

I sent my form to the address as mentioned in the form. I have made a mistake as my form does not have the signature of Parent/Guardian. All other entries in my form are completely filed except the signature of Parent/Guardian. I made payment through PNB Challan so the form contains the PNB Challan as well. Should I send my form again filled completely as I have the photocopies?
