- Notification details in PDF format will be opened at your screen check the details carefully.
- Now go back the previous page and hit on “Online Application form for Community Health Officer -Published on 31-05-2019” link.
- Fill the application form, upload documents and pay the application fee.
- At last, submit the form and assure to print a copy of the submitted form for future reference.
