Apply for Birth Certificate?
·         Parent need to download the format of birth certificate online from official website of  
·         Now fill the form and attach the required document.
·         Now verify the form from hospital’s registrar officer within 21 days.
·         Then, you need to visit the nearest District Statistical Officer and then check the details of your registration.
·         You need to pay Rs 5 for the copy of the certificate in the statistical office.

·         Application of the desired form.
·         Proof of Birth of the person in respect of whom the certificate is required.
·         Need to specifying place, date and time of birth of the person.
·         Marriage certificate of the parents
·         Copy of Ration Card/School leaving certificate, if any, showing date of birth (in the case of birth certificate only)
·         All documents to be attested by a Gazetted Officer only.
